Dear Priya,

Thank you for confirming that my full surname has now been corrected in the r=
epairs system. It is still abbreviated as =E2=80=9CAdebayo-Fitzpatrick=E2=80=
=9D in the relocation portal and the contractor used =E2=80=9CAmina Ogunleye=
=E2=80=9D when telephoning my sister. Please ensure that all three records ar=
e linked to case RCH-ADJ-2025-0417. I am concerned that this is why the panel=
 said it had not seen the occupational-therapy report.

The temporary bath board does not resolve the problem. On 27 November I could=
 not transfer back into my wheelchair after washing. I lowered myself to the =
floor and called my sister. I did not attend hospital, but I have soreness in=
 my left shoulder and have contacted my GP. I am now strip-washing at the kit=
chen sink on high-fatigue days.

Please answer the following:

1. Has the drainage and structural survey been commissioned?
2. Has the entrance-door inspection taken place?
3. What is the current date for the reconstituted relocation panel?
4. Has the equality review compared my case with the two households moved fro=
m upper-floor flats this year?
5. Why does the portal show my medical evidence as =E2=80=9Cnot verified=E2=
=80=9D when the same report was accepted for my Personal Independence Payment=
 review?

I attach no benefits documents to this email because they include my National=
 Insurance number and bank details. The welfare-rights adviser can upload red=
acted copies to the restricted area once you identify what is actually requir=
ed.

I need a substantive response, not another acknowledgement. Please communicat=
e in writing because fatigue and concentration problems make unplanned teleph=
one calls difficult.

I apologise that you were addressed by an abbreviated surname in the contract=
or call. The repairs record now contains the full name. I have asked Housing =
Options to correct the relocation portal separately.

The property team says that a drainage inspection was attempted on 18 Novembe=
r but the contractor attended without the appointment being confirmed. Their =
note records =E2=80=9Cno access=E2=80=9D. I recognise that you had not been t=
old to expect them. This should not be treated as a refusal of access.

The bathroom adaptation remains open. The transfer decision has not been chan=
ged because the panel is waiting for the feasibility report. I have asked the=
 team to confirm a new appointment and whether an interim shower chair can be=
 safely used.

My neighbour called to say someone from =E2=80=9CAccessBuild=E2=80=9D was at =
the communal door. I was at a neurology appointment and had not been given a =
date. The voicemail left for me used a different surname and said the job wou=
ld be closed if I did not call within 24 hours.

Please record that I did not refuse access. I can attend with at least 48 hou=
rs=E2=80=99 written notice. My sister can also attend if she is copied into t=
he appointment.

or flat accessible. Please confirm the survey date and whether the relocation=
 review is proceeding in parallel.
